Our medical record - Practice Fusion - is no longer using Patient Fusion.
The new portal, where you can message your provider directly is called Follow My Health. There is also an app you can download for your convenience.
If you have not received your email invitation to join our new portal-
please let us know and we will get it sent to you.
Make a Payment
You will need your Patient ID number which is located on your billing statement. If you do not know or have access to your patient ID number, please call and we are happy to provide it to you.
